Opinäytetyön tarkoituksena on kuvata työterveyshoitajan antamaa puhelinpalvelun laatua asiakkaiden näkökulmasta. Aineisto kerättiin teemahaastatteluilla. Haastattelujen kohdejoukon muodostivat Helsingin kaupungin Työterveyskeskuksen asiakkaat (n=10). Haastateltavat valittiin Työterveyskeskuksen terveydenhoitajien antamista yhteystiedoista. Yhteystiedot oli kerätty asiakkaista, jotka olivat ainakin kaksi kertaa olleet kuluvan vuoden aikana puhelimitse yhteydessä työterveyshoitajaan. Aineisto analysoitiin käyttämällä sisällön analyysia. Työterveyshoitajilta saatiin yhteystiedot kahdella eri listalla, joista haastateltavat valittiin tasapuolisesti. Tulosten mukaan osa piti soittoaikoja epäsopivana ja soittoaikojen pituutta osa haastateltavista olisi halunnut muuttaa ja pidentää. Haastateltavat arvostivat kiireetöntä puhelinpalvelua ja heidän ongelmansa huomioimista. Haastateltavat kokivat työterveyshoitajien ammattitaidon hyväksi. Terveysneuvontaa haastateltavat olivat saaneet niukasti, mutta eivät toisaalta sitä ongelmiensa luonteen vuoksi kaivanneet, koska saivat nopeasti ajan lääkärille. Opinnäytetyö antaa tietoa työterveyshoitajien puhelinpalvelun hyvästä laadusta ja asiakkaiden tyytyväisyydestä. Tuloksia voidaan pitää haastavina laadun ylläpitämisessä ja kehittämisessä.
Äldrevård är i behov av nya strukturer som bättre stödjer säkerhet och kvalitet. Det finns få studier som forskar om hur vårdledare upplever uppgiften som ledare av vårdkvalité. Studiens syfte var att beskriva vårdledares uppfattningar om kvalitetsledning inom äldrevård och -omsorg. Åtta vårdledare i Finland intervjuades. Data från intervjumaterialet analyserades med hjälp av kvalitativ innehållsanalys. Analysen genererade två teman med respektive underteman; att skapa förutsättningar för god vård med underteman; att ha välmående personal och att ha aktivt ledarskap, samt att kämpa med försämrade förutsättningar med underteman; att arbeta med otillräckliga resurser och att hantera brister hos personalen.Elderly care is expected to need structures that better support safety and quality. Few studies show how nurse managers experience their role as the leaders of quality. The aim of this thesis was to analyse nurse managers’ perceptions of quality management in the elderly care. Eight nurse managers in Finland were interviewed on the questions of quality. The data from these interviews was analysed by the means of qualitative content analysis. From this content, two themes were extracted; to create the conditions for good care, and its subthemes; to have wellbeing staff and to have active leadership, and to struggle against deterioration of conditions, and its subthemes; to work with insufficient resources and to handle shortcomings of the staff.
Objective To measure the possible magnitude of the role nurse staffing has on increasing life expectancy at birth and at 65 years old. Methods The statistical technique of panel data analysis was applied to investigate the relationship from the number of practicing nurses’ density per 1000 population to life expectancy at birth and at 65 years old. Five control variables were used as the proxies for the levels of medical staffing, health care financial and physical resources, and medical technology. The observations of 35 member countries of Organization for Economic Co-operation and Development (OECD) were collected from OECD Health Statistics over 2000–2016 period. Results There were meaningful relationships from nurse staffing to life expectancy at birth and at 65 years with the long-run elasticities of 0.02 and 0.08, respectively. Overall, the role of nursing characteristics in increasing life expectancy indicators varied among different health care systems of OECD countries and in average were determined at the highest level in Japan (0.25), followed by Iceland (0.24), Belgium (0.21), Czech Republic (0.21), Slovenia (0.20) and Sweden (0.18). Conclusion A higher proportion of nursing staff is associated with higher life expectancy in OECD countries and the dependency of life expectancy to nursing staff would increase by aging. Hence, the findings of this study warn health policy makers about ignoring the effects nursing shortages create e.g. increasing the risk of actual age-specific mortality, especially in care of elderly people.
Opinnäytetyön tarkoituksena oli kartoittaa mitkä ovat toimintaterapian keskeisiä laatutekijöitä kansainvälisten laatustandardien mukaan. Tämän selvittämiseksi tutkin Australian, Iso-Britannian, Kanadan ja Yhdysvaltojen englannin kielellä julkaistuja laatustandardeja. Opinnäytetyön tuloksena syntyi kooste toimintaterapian keskeisistä laadun tekijöistä, jotka esiintyvät tutkituissa standardeissa. Tutkimusmenetelmänä oli kartoittava kirjallisuuskatsaus. Tutkimusaineistona oli neljän englanninkielisen maan laatustandardit, joita tutkittiin yhteisten laadun määreiden löytämiseksi. Laatu on käsitteenä laaja, ja sen toteutumista voidaan arvioida monesta eri näkökulmasta riippuen tarkastelijasta. Tässä työssä tarkasteltiin laatua toimintaterapian ammatillisesta näkökulmasta. Tuloksena esitetään standardeista löydetyt laatutekijät. Löydetyt laatutekijät voidaan jakaa toimintaterapian toteuttamisen tapoja säänteleviin tekijöihin ja toimintaterapeutin osaamista säänteleviin tekijöihin. Tulosten tarkastelussa esitetään laatutekijöiden sisältöä, eli sitä, miten niitä on standardeissa määritelty. Tuloksissa tarkastellaan lisäksi miten nykyaikaisen toimintaterapian keskeiset käsitteet esiintyvät laatutekijöissä. Lopuksi tuloksia tarkastellaan suhteessa siihen, miten suomalainen lainsäädäntö ja kuntoutusta velvoittavalla tavalla ohjaava muu sääntely tukee löydettyjen laatutekijöiden toteuttamista Suomessa. Opinnäytetyötä voi jatkossa hyödyntää suomalaisten toimintaterapian laatustandardien kehittämisessä, toimintaterapeutin ammatillisten osaamisprofiilien laatimisessa ja toimintaterapian laatujärjestelmän kehittämisessä. Tässä esitetyt laatutekijät ovat yleistettävissä laajemminkin kuntoutusta ohjaavan sääntelyn kehittämiseen ja hyödynnettävissä kilpailutusten toteuttamiseen. The aim of this research is to find essencial factors of quality in occupational therapy work according to international quality standards. Quality is not easily defined, and the definition of quality is highly dependent on the standpoint of the observer or stakeholder. This study aims to observe quality on the standpoint of occupational therapy as profession and find and define common professional factors of quality. To find these factors I studied four professional quality standards, which are published in Australia, Great-Britain, Canada and USA. The result is list of factors, which define quality in these professional standards. The research method was review of literacy. The essencial factors that occur in all four standards were found and organized in two groups: factors that supervise practicing occupational therapy and factors that supervise the professional knowledge and competence of occupational therapist. As result I present factors of professional quality and how these factors are defined in professional quality standards. Finally, I present how Finnish legislation and regulation enhances and supports the presence of these factors in rehabilitation practices. These results may benefit developing national quality standards for occupational therapy in Finland, or professional competency profiles for occupational therapists. They may be utilized to develope quality systems for organizations. These essencial factors may be used to define the quality of rehabilitation also in wider context than considering occupational therapy alone.
The purpose of this study was to examine the current situation in substance abuse treatment units in Finland in taking non-Finnish speaking clients into consideration. The initiative for this research came from the Development of Alcohol and Drugs Intervention group at Stakes (National Research and Development Centre for Welfare and Health). Their aim was to gather information about the functioning and relevance of the quality assessment forms based on the quality recommendations for substance abuse work, filled in by substance abuse treatment units. The ethnic issue was chosen as the main approach in the study. The aim of this research was to answer the following questions: what is the readiness and competence in substance abuse treatment units in Finland to receive and encounter non-Finnish speaking clients, how is the quality of these services assessed and/or developed in the units, and what has been the role and functioning of the quality recommendations and quality assessment forms in working with non-Finnish speaking clients. The research methods used in the study were both quantitative and qualitative. The information concerning language services provided in the units was gathered from the quality assessment forms and basic information forms found in the database maintained by Stakes. The total amount of units found in the database was 267. In addition to that, semi-structured theme-interviews were carried out in four substance abuse treatment units in order to get a more deep understanding of how the services function in practice. The few number of non-Finnish speaking clients in the units may explain to a certain degree the results of the research. The results however showed that there is still space for improving the services. In the light of quality recommendations, the degree of language options provided in substance abuse treatment units in Finland today is low. Also the quantity of interpreter services provided in the units is scarce. There could also be unified guidelines specially tailored for substance abuse treatment units on how to work with ethnic minorities, as the knowledge is currently adopted from several different instances. The quality recommendations as well as quality assessment forms were valued and applied in the units appropriately and were also perceived to have an effect on the functioning, and quality, in the units.
