Tämän opinnäytetyön tavoitteena on antaa tietoa ikääntyvän väestön palveluohjauksesta. Toiminnallisena tavoitteena on tiedon ja moniammatillisen työn lisääminen. Tulostavoitteena on valmistaa esite palveluohjauksesta Raahen seudun kuntayhtymän työntekijöille.

Olemme tehneet kolme ryhmähaastattelua, joiden avulla kartoitamme millaista tietoa ikääntyvillä on tällä hetkellä palveluohjauksesta ja mitä he siltä toivovat. Saamiamme vastauksia olemme hyödyntäneet valmistaessamme esitettä palveluohjauksesta. Opinnäytetyömme viitekehys liittyy lähinnä ikääntyneiden palveluohjaukseen sekä palveluohjausprosessiin.

Palveluohjaus on koettu tarpeelliseksi, niin asiakkaiden kuin tilaajatahon puolellakin. Sen tuomat hyödyt ovat suuria. Kiinnostus palveluohjausta kohtaan on lisääntynyt huomattavasti väestön ikääntyessä.

Tuotteena olemme valmistaneet esitteen, joka on laadukas ja selkeä. Esitteessä esitellään palveluohjaukseen liittyvät pääasiat. Tuotteen olemme valmistaneet yhdessä toimeksiantajan kanssa. Valmis esite on informatiivinen, pieni tietopaketti työntekijöille.

The goal of this thesis is to give information of the service guidance of aging people. The functional goal of this thesis is to increase knowledge of service guidance and increase multi-professional co-operation in eldercare section. The concrete goal is to produce service guidance brochure for the employees of Raahen Seudun Kuntayhtymä.

We did three group interviews to help survey the type of information that aging people have about service guidance at the moment and what are their hopes for service guidance. Framework of this thesis conceming to service guidance process of aging people.

Based on our survey customers have a need for service guidance. The benefits that customers can achieve of service guidance is were very multi-dimensional. Interest in service guidance is increasing remarkable when people are aging.

The product of this project is high quality brochure which presents most important facts of service guidance. We created the product together with the client. The final product is informative and compact brochure for the employees in eldercare sector.
